Association Membership. LifeExec Advantage Wellness Complete is provided as part of a LifeExec Advantage Association Membership. LifeExec Advantage Association is a member organization in partnership with Health365. Members agree to be bound by the provisions and terms of the applicable Bylaws and Operative Documents, any documents required by an insurer then in effect, or any other requirements that may be adopted by LifeExec Advantage Association and Health365.
HIV/AIDS Health Restriction. By completing this enrollment application, applicants affirm no members seeking coverage within the individual or family plan have been diagnosed or tested positive for the HIV/AIDS virus.
Attestation of Accurate Information. By completing this enrollment application, applicants affirm that all information provided is precise and truthful to the best of their knowledge. Please be advised that any discrepancies or inaccuracies discovered in the submitted information may lead to the application being rejected and the denial of requested coverage.
Enrollment Deadline. All new enrollees are required to finalize their registration, including payment, by 12:00pm MT on the 22nd of each month. Should the 22nd fall on a weekend, submissions must be completed by 12:00pm MT on the preceding Friday. A complete enrollment encompasses all required member information, the appropriate premium amount, and valid payment details. While dependents may be added or modified at a later time, it is essential to process member enrollments by the stated deadline to ensure that benefits commence on the 1st of the following month.
Monthly Credit Card Billing. Credit card payments are scheduled to be billed on the monthly anniversary of the initial charge. In the event of a payment failure, please ensure to reconcile and resolve the outstanding payment by 12:00pm MT on the 22nd of the current month to maintain uninterrupted service.
Private Insurance Healthcare Program. The LifeExec Advantage Wellness Complete (AWC) program satisfies the Affordable Care Act's (ACA) requirements for minimum essential coverage. Distinguished from conventional medical insurance offerings, AWC is an innovative solution that encompasses a suite of unique benefits. These include no deductibles, complimentary medications, comprehensive long-term care coverage, and an integrated life protection program, setting it apart from traditional insurance models.
Childbirth Coverage Limitation. Please note, the LifeExec Advantage Wellness Complete program excludes coverage for non-emergency childbirth-related medical services.
Coverage Scope and Member Responsibilities. The LifeExec Advantage Wellness Complete program is able to offer a $0 deductible through the implementation of stated coverage maximums, which are aligned with national average costs and utilization patterns. It is important for members to be aware that any costs exceeding the defined coverage limits, as well as expenses that fall outside of the policy's terms, will be the member's responsibility. To ensure a thorough understanding of potential costs, coverage scope, and to make the most of their benefits, members are strongly advised to consult with a medical coverage expert before engaging the services of medical facilities.
Life Protection Plan Access. The life protection plan fund is designed to provide support during major medical events. It is not available for all or discretionary medical expenses. For detailed information on policy guidelines and how to access these funds, members are encouraged to thoroughly review their policy documentation. Should you have any claims or require assistance, please reach out directly to your designated medical coverage expert for personalized support.
Ensuring Member Data Privacy. We prioritize the privacy of our members' information. The data collected through the enrollment process is strictly used for the purpose of administering healthcare and wellness benefits. This information might be shared with our carrier partners solely for facilitating the stated benefits. Rest assured, your data will not be utilized or distributed for marketing or any unrelated purposes.
Term and Premium Stability. Our members enjoy the stability of their benefit premiums, which are secured for a full 12-month term. Although LifeExec maintains the ability to adjust prices on a yearly basis, any such changes will not occur during an active term. It's important to note that members who choose to cancel their membership before the end of their term will not be eligible to rejoin the program for a period of 24 months.
Refund and Cancellation Policy. For new members, there is a 30-day grace period from the enrollment date during which a full refund is available. To be eligible for the first 30-day refund, the request must be made before the 22nd of the first month’s enrollment. Members will retain full access to their benefits until the last day of the month, regardless of their refund request date. No refunds are available on follow on months after the payment is made. Cancellations must be made by the 22nd of the month to cancel the following month’s benefit. Members who cancel cannot reenroll in Advantage Wellness Complete for 24 months from the month of cancellation.
Program and Provider Updates: LifeExec reserves the right to modify providers and update programs as needed. Members will receive advance notification of any changes or improvements to their healthcare benefits prior to implementation.
